AJHP Style Guide : Government Docs

Tips

Publication city and state: If the city is well known, you can leave out the state. For example, you would not need to list the state for New York City, Chicago, or San Francisco.

Report number: Most government documents will include a report number on the bottom of the title page. If there is no report number, then leave this information out.
